Welcome to the New Journeys team!

We are very excited to have you on board. While it, of course, cannot summarize everything that goes into team-based first episode care, we hope this training page gives you a broad introduction to how FEP teams and our program within Washington State are structured. Included is a brief overview of the program, an Introductory Training for all new team members, Role-Specific Trainings structured to fit the needs of each specialty, Supplemental Trainings, and a collection of additional resources. Washington State New Journeys Network

Key Terms to Know

Coordinated Specialty Care (CSC)

A care model within FEP treatment in which a group of clinical experts work collaboratively with each other, with the client, and with the client's family to meet recovery goals. It is based on care models originally implemented in other countries.

Recovery After Initial Schizophrenia Episode (RAISE) Study

A large-scale clinical trial which compared treatment-as-usual to CSC treatment in the United States. CSC was found to be superior in a number of domains, as participants were more likely to stay in treatment, experience greater reduction in symptoms, greater improvements in quality of life, and participate more in work or school.

NAVIGATE

The CSC model developed out of the RAISE study. It serves as a national blueprint for CSC FEP care, and suggests teams with the following roles: Program Director/Family Education Specialist; Individual Resiliency Treatment (IRT) specialist; Prescriber; Supported Employment and Education (SEE) Specialist; Peer Support Specialist; Case Manager.

New Journeys Team

These are the CSC FEP teams in Washington State. They are modeled off of the NAVIGATE model, with additional training and tools from UW training team. They participate in monthly case presentation and didactics through an online ECHO clinic.

Admission Criteria

Eligibility

Age 15-40 years old

 

Diagnosis of a primary psychotic disorder (e.g., schizophrenia, schizoaffective disorder, schizophreniform disorder, brief psychotic disorder, or other specified schizophrenia spectrum and other psychotic disorder) or an affective psychosis disorder (e.g. bipolar disorder with psychotic features, major depression with psychotic features)

 

Psychotic symptom duration of at least one week but < 2 years​​​

​

The psychosis is not known to be caused by:

 

A current diagnosis of a pervasive developmental disorder and/or autism spectrum disorder, psychotic disorder d/t another medical condition, or substance/medication-induced psychotic disorder

 

or an IQ < 70

Consultation Program

The UW Implementation Team provides teleconsultation via role-specific Consultation Calls and network-wide ECHO Clinics.

Consultation Calls

Each trainer on the UW Implementation team facilitates ongoing Consultation and training for each role on a New Journeys team via Zoom. These Consultation calls provide a forum not only for continuing education, but a place for vetting problematic, challenging clinical cases and discussing topics and challenges more focused and relevant to individual roles that may otherwise be overshadowed in network-wide consultation. Once you are on-boarded to your team, you will be likely receive an introduction to these consultation calls from your role's trainer. FEP ECHO Clinics

Monthly FEP ECHO Clinics are a network wide consultation forum for all New Journeys teams. Each ECHO Clinic takes place on the 4th Wednesdays between 12pm - 1:30pm. Each FEP ECHO clinic consists of introductions and network wide announcements, a 30-minute didactic presentation on a pre-determined topic (often by a guest presenter), and a Case Presentation by a New Journeys team (selected on a rotating schedule) followed by case recommendations by the trainers and other New Journeys team members. Check out the ECHO Clinics tab at the top of this page for the current schedule of didactic topics and presenting teams. 

SCID-5 & Differential Diagnosis:

This is a more intensive and highly diagnostic training designed for team members engaged in screening participants in the program (usually the Program Director, though occasionally the IRT or Psychiatric Care Provider). 

New Member Checklist

 

You should receive a welcome email from the training team shortly after joining which will introduce you to your consultant and provide more in-depth guidance regarding trainings for your role. While the process of each role is unique, we've included a generalized checklist to help you navigate the onboarding process.

Confirm that you are on the appropriate mailing distribution lists by emailing fepecho@uw.edu.

 

Visit the ECHO Clinic and Consultation Call tabs to view the upcoming calendar, and add the dates to your calendar.

 

Complete the Introductory Training and/or sign up for a Synchronous Training.

 

Complete your Role-Specific Training(s)​ and/or sign up for a Synchronous Training.*
 

​​Please Note:

 

Case Managers: Currently there are no role-specific trainings for case managers.

 

IRT Clinicians: IRT's should take this e-Primer on Cognitive Behavioral Therapy for Psychosis (CBTp) prior to completing the role-specific training.

 

Nurses: While there are no role-specific trainings, nurses may find it very beneficial to take the Psychiatric Care Provider Training

 

Peer Specialists: The Peer Training was not recorded to maintain privacy and facilitate open and honest conversation. 

 

Program Directors: Program Directors should also complete the SCID-5 & Differential Diagnosis supplemental training. Those who are also Family Education Specialists should also complete the Family Education Specialist Training.

Orientation to Psychosis and FEP: This training is recommended for those with little or limited background in psychosis or FEP. This training is open to all New Journeys team members. During this training Dr. Melton will review the basics of psychosis, including common symptoms, how they typically manifest, as well as common diagnostic categories for First Episode Psychosis.​

 

SCID-5 & Differential Diagnosis: This is a more intensive and highly diagnostic training designed for team members engaged in screening participants in the program (usually the Program Director, though occasionally the IRT or Psychiatric Care Provider).
